French home-care nurse: rounds and care act pricing

A prescription reaching its deadline without anyone noticing, a round organised from memory, a care act priced by guesswork because the French NGAP scale is not reliably written down anywhere: the daily life of a self-employed nurse practice in France runs on several small notebooks. This application brings together patient files, rounds and care act pricing, through to the billing statement.

Who the application is for

For self-employed nurses practising in France, alone or in a shared practice, who manage patient files, daily rounds and the pricing of their care acts under the French NGAP scale in force.

What the application does

  • Keeps one file per patient — attending doctor, insurer, next of kin or payer — applying a data-minimisation policy: no social security number and no full medical care record.
  • Records prescriptions with their validity period and care frequency, and automatically alerts when a prescription reaches its deadline without a renewal.
  • Plans a daily round with several patients ordered by sequence, with the planned and the actual time of each visit.
  • Prices each care act against a fully configurable French NGAP grid — key letters, surcharges, act codes — that the practice enters and keeps up to date itself; the amount is calculated and then fixed to the act.
  • Calculates the travel allowance for each visit against a configurable scale, either per kilometre or a flat rate, then fixes it on the visit.
  • Consolidates a patient's completed care acts over a period into a billing statement, generates the corresponding invoice and prints a summary document.

Day to day

  1. Check and complete your French NGAP pricing grid (key letters, surcharges, act codes and travel scales) against the agreement in force, before first use.
  2. Open a patient's file, enter their attending doctor and payer, then record their prescription.
  3. Plan the day's round with the order in which patients are to be visited.
  4. Record each care act performed with its priced code; the amount is suggested from the grid, then fixed to the act.
  5. At the end of the period, consolidate a patient's billing statement, confirm it, then generate the corresponding invoice.

What the application does not do

  • It does not electronically submit anything through the French Sesam-Vitale system: that requires an accreditation and a card reader that the application does not provide.
  • It does not update any value of the French NGAP scale on its own: the grid must be entered and kept up to date by the practice, and the rates change with each agreement amendment.
  • It does not keep a full nursing care record in the regulatory sense (macro-target, targeted transmissions).
  • It does not resolve the question of health-data hosting: depending on where your instance is hosted, you may be subject to the French Health Data Hosting certification, and it is for you to check this, if needed with a lawyer or your hosting provider — the application does not replace that advice.

Free and full editions

The free edition already covers patient files, prescriptions with their alert, the full NGAP pricing of care acts, and round organisation with the running total of kilometres. The full edition installs on top without losing anything, and adds the costing of travel allowances and the billing statement with its PDF export and invoice.

Getting started

Before any pricing, complete your key letters, surcharges and travel scales against the national agreement in force, which are delivered empty on installation. Then check your own situation regarding health-data hosting before going live.
